Applying for WIC in Louisiana

Louisiana WIC offers an online referral application, phone scheduling, and in-person appointments. All individuals applying for WIC must be present at their certification appointment โ€” bring infants and children with you, and bring one clean, dry diaper for weighing any infant.

Step 1: Apply Online or Call

  • Apply online: Submit a participant referral form through the myWIC app or at mywic.us/participantreferral?program=LA. After submitting, a WIC clinic will contact you to schedule your certification appointment.
  • Call the BABY line: Dial 1-800-251-BABY (1-800-251-2229) to find the WIC clinic nearest you and schedule an appointment by phone.
  • Visit louisianawic.org: Use the clinic locator to find your nearest clinic and contact them directly.

Step 2: Gather Your Documents

Bring the following to your appointment โ€” or upload documents through the myWIC app in advance (bring physical copies as well, in case uploaded images are unclear):

  • Proof of identity for yourself and each person applying โ€” a driver's license, state ID, passport, birth certificate, or hospital birth records for infants; a Louisiana Medicaid card is also accepted
  • Proof of Louisiana residency โ€” a utility bill, current lease, processed mail at your address, Medicaid card, voter registration card, or current pay stub showing your address
  • Proof of income โ€” pay stubs, Social Security award letters, or other income documentation for the prior 30 days. If you receive Medicaid, SNAP, or TANF, bring that enrollment documentation instead โ€” it replaces income proof entirely.
  • For infants: one clean, dry diaper for weighing at the appointment
  • For foster children: foster care or adoption papers

Step 3: Your Certification Appointment

At your appointment, a WIC health professional โ€” including access to Registered Dietitian Nutritionists โ€” will verify documents, conduct a health and nutrition assessment including height, weight, and hemoglobin levels, determine your food package, and issue your eWIC card. Louisiana WIC also offers virtual nutrition education classes in addition to in-person options through wichealth.org, with over 20 lessons available 24/7 in English and Spanish.

Note: Louisiana WIC benefits are not automatically loaded each month like SNAP. Benefits are added to your card during WIC appointments, so keeping your appointments is important for maintaining uninterrupted benefits.

The Proxy Option

If attending WIC appointments in person is difficult due to work, school, transportation, or other barriers, Louisiana WIC allows you to designate a proxy to attend certain appointments and shop for WIC foods on your behalf. Fill out the Proxy at Certification Form and bring it to your next appointment. Ask your local WIC clinic for the form and instructions.

Recertification

Pregnant women are certified through pregnancy and six months postpartum (twelve months if breastfeeding). Infants are recertified around six months and approaching their first birthday. Children are certified every six to twelve months until their fifth birthday. The myWIC app displays your certification status and appointment information.
