Applying for WIC in Indiana

Indiana WIC does not have a statewide online application. To apply, you need to contact the WIC clinic in your county to schedule an in-person certification appointment. There are clinics in all 92 Indiana counties across the state's network of 174 WIC clinic sites.

Step 1: Find Your Local Clinic

There are three ways to find the WIC clinic nearest you:

  • Visit in.gov/health/wic and use the county clinic locator
  • Download the INWIC app (search "IN WIC" on the App Store or Google Play) and tap "Find WIC Clinic"
  • Call the Indiana WIC information line at 1-800-522-0874, Monday through Friday during business hours
  • Email inwic@health.in.gov with your county or zip code

Step 2: Gather Your Documents

Bring the following to your certification appointment:

  • Proof of identity for yourself and any children applying โ€” a driver's license, state ID, birth certificate, or other government-issued document
  • Proof of Indiana residency โ€” a utility bill, lease, official mail, or any document showing your current Indiana address
  • Proof of income โ€” a recent pay stub, employer letter, or documentation of Medicaid, SNAP, or TANF enrollment (if you receive any of those, that alone satisfies the income requirement)
  • Immunization records for any children under age 2 applying for WIC
  • For pregnant applicants: documentation from a healthcare provider confirming pregnancy

Step 3: Attend Your Certification Appointment

At the appointment, a WIC health professional will conduct a brief nutritional and health assessment to confirm nutrition risk โ€” the federal requirement for WIC enrollment. Staff will verify your documents, record your household information, and determine your food package. If eligible, you leave with your eWIC card loaded with your first month of benefits.

Indiana WIC also makes referrals at certification appointments โ€” if staff identify that you may qualify for Medicaid or other social services, they will connect you with those programs as part of the same visit.

Transportation Assistance

Some Indiana WIC clinics offer transportation assistance for appointments. Ask your local clinic what options may be available in your area, particularly if transportation is a barrier to attending your certification or recertification appointment.

Recertification

WIC certification periods vary by participant type. Pregnant women are certified through pregnancy and six months postpartum (or twelve months if breastfeeding). Infants are recertified around six months and again near their first birthday. Children are certified every six to twelve months until age five. The INWIC app displays your certification end date and will alert you before benefits expire. Contact your clinic before your certification period ends to schedule a renewal appointment.

If you move within Indiana, contact your new county's WIC clinic to transfer your benefits โ€” your certification period and food package transfer with you. If you move out of Indiana, ask your current clinic for a WIC Verification of Certification (VOC) card before you leave. Take that card to a WIC clinic in your new state to continue benefits without starting the certification process from scratch. You cannot use your Indiana eWIC card in another state.
