Arizona WIC is administered by the Arizona Department of Health Services (ADHS) through authorized vendors and clinics across the state. The program serves all Arizona counties — from densely urban Maricopa County to remote tribal communities across the Navajo Nation and other reservations.
Arizona WIC has its own dedicated shopping app called EZWIC (also written EzWIC), developed by the Arizona Department of Health Services and available free on iOS and Android. The EZWIC app lets participants scan barcodes in the store, check benefit balances, and manage their WIC shopping.
Who Qualifies for WIC in Arizona
Arizona WIC is open to pregnant, breastfeeding, and postpartum women, infants, and children under the age of five. Foster and adopted children who meet the eligibility criteria are also eligible — Arizona ADHS publishes specific guidance for foster and adoptive families. All kinds of family structures are welcome: single parents, married couples, working families, and families living with extended relatives.
If your household participates in AHCCCS (Arizona's Medicaid program), SNAP (food stamps), or TANF, you automatically meet WIC's income requirement. Bring your current enrollment documentation to your appointment. For households not in those programs, the income threshold is 185% of the federal poverty level based on gross household income.
What Arizona WIC Provides
Benefits are loaded onto an Arizona eWIC card redeemable at authorized grocery stores, pharmacies, and commissaries statewide. Arizona's food packages include fruits and vegetables, milk, yogurt, cheese, soy milk, eggs, whole grains, cereals, infant formula, and more. Some items — including non-contract or soy-based infant formulas and specialty formulas — require a completed Food and Formula Request Form from a medical provider before Arizona WIC can issue them.
Every participant receives a Cash Value Benefit (CVB) for fruits and vegetables. Arizona also participates in the WIC Farmers Market Nutrition Program (FMNP), which provides eligible WIC households with coupon booklets for purchasing fresh, Arizona-grown produce at participating farmers markets and farm stands. Recertification in Arizona
Certification periods in Arizona vary by participant category — pregnant, postpartum, breastfeeding, infant, or child. Your local WIC clinic will set your certification period at your initial appointment and notify you when recertification is due. Automatic Income Eligibility in Arizona

What Counts as Income in Arizona
Arizona WIC counts all gross household income before taxes, including wages, self-employment income, Social Security, child support, alimony, and unemployment benefits. Bring the most recent month of pay stubs for everyone working in the household. Housing assistance and tax refunds are not counted as income.
Income Is Not the Only Requirement
Meeting the income threshold qualifies you on one criterion, but Arizona WIC also requires a nutritional risk determination. You will be asked questions about your health and diet at your WIC appointment, and a staff person will assess nutritional risk at no charge. Arizona WIC Food Package Overview
Dairy and alternatives: Arizona WIC includes milk, yogurt, cheese, and soy milk in its food packages. Soy milk is available as an alternative to cow's milk. Some non-contract or soy-based infant formulas require a completed Food and Formula Request Form from a medical provider.
Whole grains: Approved options include whole grain bread, cereals, brown rice, whole wheat pasta, and other whole grain products. Your monthly allowance is measured in ounces and you can mix and match approved grain products.
Fruits and vegetables: The Cash Value Benefit (CVB) covers fresh, frozen, and canned options. In addition to the monthly CVB, eligible Arizona WIC households may receive FMNP coupon booklets for fresh, Arizona-grown produce at participating farmers markets and farm stands — ask your WIC clinic about current FMNP availability and how to apply.
Special formulas: Arizona WIC can provide special formulas or nutritional products for participants with certain medical conditions, but a prescription from a physician or healthcare provider using the Food and Formula Request Form is required before these can be issued.
The EZWIC App
The EZWIC app is Arizona's dedicated WIC shopping tool, developed by ADHS and available free on iOS and Android. Once you register your eWIC card, the app lets you scan barcodes in the store to see if a food is approved and shows your available benefit balance. It also provides appointment reminders, food benefit expiration notifications, and breast pump return reminders. The app includes a clinic search with filters to find clinics open on specific days (Arizona-only feature), a store locator, and the ability to submit complaints against WIC participants, staff, or vendors directly from the app. Food list and balance information is available in multiple languages through the app's "Other Resources" menu.
WIC in a Click
Arizona WIC's "WIC in a Click" online module and podcast system allows participants to complete WIC nutrition education appointments on their own schedule at azdhs.gov/prevention/azwic/wic-in-a-click. Modules and podcast episodes cover nutrition, breastfeeding, postpartum wellness, feeding your family, and more — and can be completed any time, day or night. WIC in a Click is also used by WIC programs in American Samoa, CNMI, Guam, the Navajo Nation, and Washington DC. After completing a module or podcast episode and the required questionnaire, benefits are issued to your eWIC card within two to three business days.
Arizona WIC Farmers Market Program
Arizona WIC participates in the WIC Farmers Market Nutrition Program (FMNP). Eligible households receive FMNP coupon booklets for fresh, Arizona-grown fruits, vegetables, and herbs at participating farmers markets and farm stands. Contact your WIC clinic for current benefit amounts, application instructions, and availability — FMNP vouchers are distributed on a first-come, first-served basis while funding lasts.
