Applying for WIC in Arizona

Arizona has an online WIC application process starting at azdhs.gov/prevention/azwic. The ADHS website includes a video guide on how to start your application and a clinic locator to find the nearest office. You can also apply directly at myazwic.com โ€” Arizona's WIC participant portal. Call 1-800-252-5942 Monday through Friday, 8 a.m. to 5 p.m., to find a clinic or get application help.

Before Your Appointment

  • ID for every person enrolling โ€” adults need a valid driver's license or other picture ID; infants need a crib card or birth certificate; children need a birth certificate or immunization record
  • Proof of Arizona address โ€” utility bill, bank statement, or other bill that goes to your street address. P.O. boxes are not accepted
  • Proof of income or program participation โ€” if you receive AHCCCS, SNAP, or TANF, bring a recent letter from that program showing current eligibility. If you don't receive those programs, bring the most recent month of pay stubs for everyone working in the household

What Happens at Your Appointment

At your Arizona WIC appointment, a staff person will review your rights and responsibilities as a WIC participant, ask questions about your health and diet to assess nutritional risk, and review your income eligibility. You'll receive your eWIC card loaded with food benefits and a review of the WIC food list and how to shop. Any nutrition and exercise questions or goals for your family will be discussed.

WIC in a Click โ€” Complete Education on Your Schedule

Arizona WIC's "WIC in a Click" system lets you complete nutrition education appointment requirements online at any time โ€” day or night โ€” at azdhs.gov/prevention/azwic/wic-in-a-click. After completing a module or podcast episode and the required questionnaire, your WIC benefits are issued to your eWIC card within two to three business days. This significantly reduces the number of in-person clinic visits needed after initial certification.

The EZWIC App

Once enrolled, download the EZWIC app (free on iOS and Android). Register your eWIC card in the app for full features: scan barcodes in the store to check if items are approved, view your benefit balance, receive appointment reminders and benefit expiration alerts, find WIC clinics open on specific days, and locate authorized WIC grocery stores. WIC Farmers Market Program

Once enrolled, ask your WIC clinic about the WIC Farmers Market Nutrition Program (FMNP). Eligible households can receive FMNP coupon booklets for fresh Arizona-grown produce at participating farmers markets and farm stands. Recertification in Arizona

Your certification period length depends on your participant category โ€” pregnant, postpartum, breastfeeding, infant, or child.
